Urgent Care and Walk-in Clinic visits carry $25 and $15 copayments; 'Copayment is not applicable if the Declaration states a $0 Deductible' (p. 2).

Refund if the visa is refused

Not stated as a refund ground. The certificate's cancellation clause grants a full refund only within a three-day 'Review Period' from the Initial Effective Date (§C(13), p. 10). After it, with no claims filed, 'a cancellation fee of fifty dollars ($50.00 USD) will be charged, regardless of the reason for cancellation' and 'only Premium covering time periods after the requested cancellation date are refundable'; once a claim is filed the Premium 'is fully earned and is non-refundable' (p. 11). Visa refusal appears nowhere in the certificate, the plan description or IMG's FAQ — all read August 31, 2026.

Certificate delivery

The certificate describes itself as 'an outline and evidence of eligible coverages and benefits payable' and states 'This Certificate is not part of the insurance contract.' (§B, p. 7; definition, p. 29); the Declaration issued with it is the document 'evidencing the Insured Person's insurance coverage' (definition, p. 30). IMG's FAQ states: 'IMG will process your application and send your ID card and other documents within one business day' (imglobal.com/faq, read August 31, 2026). Neither the certificate, the plan description nor the FAQ mentions a visa letter or an embassy-facing certificate.

Naming: IMG's plan description sells this cover as 'Patriot International®' and links this document as 'Patriot International - Sample Insurance Contract'; the certificate itself is titled 'Patriot International Lite', and its figures match the plan description exactly ($50,000-$1,000,000 options, deductibles $0-$2,500, 'Renewable up to 24 months'). Patriot International Platinum is a separate document, not read for this row. Inpatient hospitalization requires pre-certification; for an emergency admission 'Pre-certification must be completed within forty-eight (48) hours after the admission' (§E(4), p. 15), and non-compliance reduces eligible expenses by 50% (Benefit Summary, p. 1). Eligibility (§D, p. 14): on the effective and renewal dates the insured 'must have legally departed the Country of Residence and legally entered the Destination Country'. Benefit Period: 'Charges incurred in the United States are not eligible for coverage during the Benefit Period' (p. 1).
